GENERAL FUNCTIONS :
The Case Manager is responsible for the initial and ongoing assessment and evaluation of clients’ need for supportive and essential services throughout their course of care. With treatment and residential care as the primary focus of the client’s activities; the Case Manager will take direction from a primary therapist and program coordinator under the oversight of Assistant Clinical Director in order to appropriately assess, and develop a client-focused individualized plan to address client’s basic and supportive service needs.
DUTIES AND RESPONSIBILITIES :
- Develops and maintains a directory of the community resources utilized for client referrals, including contact persons, description of services and referral
procedures.
Develops and maintains effective working referral relationships with key stakeholders, service professionals and community partners in order to coordinatesupportive and essential services for clients.
Conducts program intake and orientation to program services with clients.Conducts thorough needs assessment with each client as assigned, prioritizing needs including but not limited to housing, employment / education / vocationaltraining, childcare, legal assistance, clothing, financial / credit counseling and support, primary physician and transportation choices and / or supports. May also
connect client to peer recovery coaching as needed.
Must assess, determine and appropriately document housing stability utilizing accepted agency procedures; provide follow-up services / appointments andreporting as required.
Assists clients with addressing basic needs and overcoming barriers to accessing financial support (employment, SSI / SSDI or other benefits), safe housing,regular healthcare and addressing legal issues as needed.
Supports an integrated approach to substance use disorder treatment through coordination of care services and maintaining effective communication withprimary counselors and the treatment team. Any previously unidentified or unresolved medical or clinical problems that come to the attention of the Case
Manager is communicated to the program coordinator and primary counselor.
Possesses strong interpersonal communication skills and ability to effectively communicate with supervisory staff, peers, and others to assure appropriate use oftime and resources.
Advocates for clients to remove barriers to access for supportive and essential services.Maintains up-to-date documentation regarding each client’s goals and progress toward engaging with supportive and essential services. Minimally this includesthe initial assessment, person centered case management plan with updates, progress notes, and a discharge summary.
Mandatory participation in scheduled committees and work groups aimed at improving coordination of services, access to services and related issues, includingbut not limited to the Greater Lansing Homeless Resolution Network and / or Human Services meeting(s).
Attends and participates in ongoing staff development activities that support professional development and enhancement of skills in case management.Communicates with clinical staff about case management and its role and function in the integrated care; addressing clients basic needs.Promotes MMRS philosophy and mission modeling appropriate behaviors, life skills, and effective communication and leadership traits personally andprofessionally. Maintains professional boundaries and ethics in accordance with strict healthcare regulations and MMRS Policies.
Interacts with staff, clients and community members demonstrating cultural competency, respect, dignity and integrity with the ability to interact effectively with adiverse population.
Strictly adheres to all federal and state laws, rules, regulations and MMRS policies and procedures regarding the confidentiality of client information and clientrights.
Participates in program staff meetings, in-service training's and other agency activities as requested and assigned.Complies with agencies conflict resolution protocols.Essential service health care provider. Additional stress and risk during critical situations commensurate with our license and scope of practice to protect thehealth and safety of clients.
Employer retains the right to change or assign other duties to this position.QUALIFICATIONS :
Minimum of a Bachelor’s degree in social services or related human service field. Prefer experience or additional course work in areas of substance use and co-occurring mental health disorders. Must demonstrate knowledge of the dynamics of substance use disorders.
If recovering from a substance use disorder, must have a period of demonstrated continuous sobriety and stability in maintaining healthy lifestyle for at least two continuous years.
