Essential Job Results:
- Provide individualized skill-building and coping interventions for adults with Serious Mental Illness and/or Substance Use Disorder, focusing on clients with significant functional deficits.
- Deliver community-based case management services, including forming treatment teams, setting treatment goals, monitoring progress, and developing sustainable recovery plans.
Main Job Tasks & Responsibilities:
- Coordinate case management efforts for individuals at risk of decompensation due to Serious Mental Illness and/or Substance Use Disorder.
- Document case management activities accurately in the Electronic Health Record (EHR) system.
- Offer recovery-oriented education and materials to clients to develop coping skills.
- Implement evidence-based practices such as Critical Time Intervention, DBT, CBT, and Motivational Interviewing.
- Develop Individual Treatment Plans within 30 days of admission, outlining steps for stabilization, resource acquisition, and relapse prevention.
- Advocate for clients by engaging with community members and participating in coalitions and stakeholder meetings.
- Establish and maintain relationships with referral sources including healthcare professionals, law enforcement, probation officers, and social service agencies.
- Collaborate closely with the clinical treatment team to ensure coordinated care.
- Provide safe transportation for clients to appointments related to treatment goals.
- Assist with crisis calls and drop-in supports as needed.
- Participate actively in staff meetings and training sessions.
- Fulfill additional tasks as assigned by supervisor to support the organization's objectives.

Required Qualifications:
- Preferred: Bachelor’s Degree in a Human Service related field.
- Alternatively: Associates Degree or 2-5 years of experience in a Human Services-related field.
- Strong organization, time management, verbal de-escalation, and communication skills essential.
