What are the responsibilities and job description for the Community Health Worker - Community Action Reentry Services -Bilingual Required position at Mid-Willamette Valley Community Action Agency?
The Bilingual Community Health Worker (CHW) will work collaboratively to assist our formerly incarcerated clients reach optimum health and wellness. The CHW provides care coordination that includes assessment, planning, implementation, monitoring, advocacy and linkage to available services and resources. This position requires Oregon Health Authority (OHA) approved Community Health Worker (CHW) certification within 12 months of hire and on-going recertification every three years. The person in this position will focus on increasing behavioral supports including peer outreach, education, prevention, and increase resource connection to our formerly incarcerated Marion & Polk County community members.

MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required of this position.
EDUCATION and/or EXPERIENCE
Associate Degree.
Two years of experience working in human services, public health, corrections, or work with adults with barriers.
An equivalent combination of education and experience may be considered.
KNOWLEDGE, SKILLS & ABILITIES
Basic proficiencies in computers, MS Office products, database software and web tools.
Has experience and is comfortable working with a diverse population.
Demonstrates tact, diplomacy and empathy when communicating formally and informally with clients, staff, and program partners.
Successful multitasker that can adapt and work in a flexible environment.

Bilingual English-Spanish skills are required for this position.
- To qualify as bilingual, staff must pass a bilingual exam demonstrating fluent speaking, reading, and writing in English and Spanish. The posted hourly rate is inclusive of a 2% bilingual differential.
CERTIFICATES, LICENSES, REGISTRATIONS
Driver qualifications are required for this position. Valid driver's license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
Candidate must pass a comprehensive MWVCAA background screening prior to employment.
Basic Life Support/First Aid Certification is required within first 30 days of hire.
Candidate must pass pre-employment and random drug screening.

Motivates and encourages individuals to obtain care and services to achieve their wellness needs.
Provides social support and builds appropriate social networks.
Provides information about the Oregon Health Plan (OHP), medical services, housing, education and other related services.
Presents information at agencies and at community events to increase health and wellness literacy. Shares culturally appropriate and accessible health education and information.
Develops and implements outreach strategies to connect clients with community health resources and services.
Conducts follow-up to ensure health coverage and medical needs are met.
Enters data into multiple database which includes client tracking, follow-up, and support as required.
Respects confidentiality at all times.
Understands how to identify common barriers that clients face and the appropriate resources to assist in removing or minimizing those barriers.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.

Working with coworkers/clients over the phone, via video conferencing, and in-person.
Occasional noise and distractions in work spaces. May experience loud, agitated, unpredictable behaviors from clients.
