Job Title: RN Case Manager (Discharge Planning)
Unit: MS/Tele
Duration: 13 Weeks (With possibility of extension)
Shift: 5 x 8 Hour Days (8:00 AM - 4:30 PM)
Guaranteed Hours: 40
Gross Weekly: $3000/Week
(W2) Taxable: $1229/Week
Non-Taxable: $1771/Week
Responsibilities
- Expected to have knowledge of discharge planning and making referrals.
- Will typically be working on a MS or Tele unit and will split the floor with a Social Worker.
- Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning.
- Expected to round on the unit with physicians/team.
- Need to have a positive attitude, be able to critically think, and good time management skills.
Salary : $3,000
