Job Summary:

Providence Home Health Alaska is seeking a skilled and compassionate Registered Nurse (RN) - Home Health to join our team in Anchorage, AK. This role involves providing high-quality, patient-centered care in a home health setting, ensuring patients receive the necessary medical attention while maintaining their independence at home.

Responsibilities:

• Deliver comprehensive nursing care to home health patients in accordance with established policies and procedures.
• Conduct patient assessments, monitor health status, and develop individualized care plans.
• Administer medications, treatments, and provide wound care as required.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity of care.
• Educate patients and their families on disease management, treatment plans, and preventive care strategies.
• Complete required documentation using EPIC charting system.
• Travel to patient homes within Anchorage and surrounding areas (occasional maximum radius of 50 miles).
• Maintain compliance with OASIS documentation requirements.
• Ensure adherence to all infection control and safety guidelines.

Qualifications & Requirements:

• Experience: Minimum 1 year of skilled home health experience required.
• Certifications: Must have an active Basic Life Support (BLS) certification.
• License: Must hold an active RN license (Pending licenses accepted).
• COVID-19 Vaccination: Primary series and at least one booster required.
• Transportation: Must have reliable personal transportation. Mileage reimbursement follows IRS standard rates.
• Local Candidates: Accepted (within 25-50 mile radius).
• First-time Travelers: Accepted.

Additional Job Details:

• Patient Ratios: 1:1 (May visit up to 6 patients per day, with a standard of 30 productivity points).
• Weekend Requirement: No weekends required.
• On-Call Requirement: No on-call shifts required.
• Floating Requirement: No floating between units.
• Block Scheduling: Not accommodated.
• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks).

Compensation & Benefits:

• Payrate: $3,006.00/week
• Overtime Pay: 1.3x after 40 total hours in a week
• Holiday Pay: 1.3x for designated holidays
• Charge Nurse Pay: Additional $2.50/hour
• On-Call Pay: $15.00/hour
• Call-Back Pay: 1.3x standard rate
• Free Parking on-site
