What are the responsibilities and job description for the Coordinator of Community Services position at Middle Flint Community Service Board?
Description of Duties: The CCS will provide oversight of all levels of case management including non intensive case management, Intensive Case Management and Community Support Team. The goal of CCS is to lead all levels of case management with support, supervision, monitors productivity, and daily flow of case management. Provides oversight of non-intensive case management (NICM), ICM and CST. Receives referrals, case history, assessments, and treatment goals to assign to case managers. Provide effective on-going interventions/services that enhance individuals’ ability to live a quality lifestyle that promotes stability and independence in their natural environment. Determines team caseload by the level of acuity and needs of the individuals served. Facilitates weekly team meetings of NICM to address changes in caseload, policy updates, high risk individuals and needs in providing person-centered clinical care etc. Manage program budget and contract obligations. Ensure program fidelity and integrity in compliance with DBHDD provider manual. Attend some weekly ICM and CST meetings. Ensure follow up of referrals in a timely manner. Communicate with other service providers to ensure treatment information is shared seamlessly. Maintains confidentiality of client-related information. Complete electronic documentation within 24-48 hours after delivery of services. Review failed activities and failed claims reports to ensure compliance. Complete and submit required reports to clinical director and other departments. Perform other duties as assigned. Transporting individuals served is a requirement.

Minimum Qualifications: Relevant master’s degree in the counseling field, e.g., Social Work, Mental Health Counseling, Marriage and Family Therapy, etc. Must be licensed by the Georgia Composite Board to practice independently as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), and Licensed Marriage and Family Therapist (LMFT). Must have a valid unrestricted Georgia Driver’s License. Must be keyboard proficient. Two or more years’ experience working with individuals in community and home settings.
Preferred Qualifications: A minimum of two years of managerial experience of professional level and support staff. In Depth comprehension of health care policy, administration, and systemic implications. Knowledge of DBHDD role and functions in statewide healthcare delivery systems.
