Position Title: CPS Mental Health Group Facilitator, FT
Designation: Mission Critical Position
Minimum Entry Salary *: Negotiable Based on Experience
Location: Warner Robins, GA
Services Area/Programs: Transformations/Peer MH
Description of Duties:: Ensure Failed Days for Failed Activities are three (3) days or less. Ensure Failed Days for Failed Claims are seven (7) days or less. Maintain a monthly peer unit authorization of 95% or better. Ensure there is no lapse in authorization of services. Meet expected monthly and quarterly productivity standards as required. Complete all documentation within 24-48 hours from date of service delivery. Monitors client’s progress towards goals, provides crisis interventions as needed. Provides one on one peer services, peer groups as needed based on client needs. Engage in community outings with clients and staff. Assist with transportation of clients and ModivCare billing. Assist with monitoring clients during day treatment hours. Ensure linkage to all needed/required clinical services as requested by client and treatment team. Facilitate and/or coordinate group activities in accordance to service definitions. Complete daily/quarterly vehicle inspections and follow process for submission to Corporate Compliance Dept. Share “lived experience’ with others individually and in a group setting. Provide advocacy for appropriate services and resources. Transporting individuals served is a requirement.

Qualifications: High school diploma or GED AND One year experience in a social services related position OR Two years at an accredited college or university with at least 15 credit hours in a social services related field. Must be CPS-MH or obtain CPS MH Certification within 2 years of employment.
