Middle Park Health is a Critical Access Hospital with MedSurg Swing/Extended Skilled Care Unit in Kremmling, CO where we seek RNs with an affinity for geriatric and rehab skilled care. Day and Night shifts are available.
Kremmling, CO in Grand County is home to more than you could ever find time to do! POSITION PURPOSE:
The Registered Nurse providing care to the patient population(s) is responsible for assessing, planning, implementing and coordinating patient care in Skilled Nursing, and Long Term Care settings while utilizing accepted standards & protocols of Advanced Cardiac Life Support (ACLS). The Registered Nurse is responsible for the achievement of patient outcomes through the implementation of clinical pathways.
Essential Job Duties:
- Utilizing the Nursing Process the RN provides direct clinical nursing care and services to the inpatient population, monitoring, measuring patients’ progress towards goals, and redirecting care based on assessment of individual client needs to successfully achieve desired outcomes.
- Demonstrates high attention to providing thorough and detailed documentation of all patient care activities, assessments, interventions, treatments, evaluation, and redirection.
- Collaborates with members of the multidisciplinary team and directs interactions which are specific to needs of the individual patient.
- With the patient’s permission and as appropriate, involves the patient’s support systems / family, in the development of the individualized plan of care, providing information and feedback to assure optimal outcomes and adequate preparation towards discharge.
- Redirects care and goals as needed to meet the physiological, psychological, emotional, cultural, and spiritual needs of the client in a safe environment.
- Coordinates nursing interventions to enhance achievement of expected outcomes and to ensure readiness for discharge
Requirements:
- High school graduate or GED equivalent.
- Licensure: licensed as a professional Registered Nurse in the State of Colorado.
- Graduate of an Accredited School of Nursing preferred.
- 6 months of Medical – Surgical Nursing / Skilled Nursing or equivalent (desired)
- Basic Life Support – BLS (or within 30 days of hire).
- Advanced Cardiac Life Support – ACLS (or within 6 months of hire).
Status: Full-Time, Part-time or PRN. We have day and night shifts available .
Compensation: Base hourly rate $33.00 -$55.00 DOE shift differentials. FT permanent RN's receive a $10,000 sign-on and are eligible for $350/mo housing stipend if resident of Grand, Summit or Jackson county.
