The position of Community Support Specialist is responsible for:
- Provides support services to consumers who demonstrate functional impairments in major life activities due to mental, behavioral or emotional illness or has difficulty establishing or maintaining normal interpersonal relationships to such a degree that he/she is at risk of hospitalization or homelessness because of conflicts with family or community. Expected to generate a minimum of 22 units if working in the community and 25 units if working in an Assisted Living facility.
- Provides medication management; assists consumers with self-administered medications and monitors medication compliance. Aids consumers in attending to personal care and hygiene needs; assists consumers in personal banking and shopping needs; attends community activities with consumers; acts as advocate for consumer in community.
- Fosters hope for recovery through education and self-esteem building exercises.
- Assists in the development and implementation of individuals support plans; to include training on food preparation, household chores, and laundry; assists in menu preparation and the planning of scheduled activities to include recreational, social, and vocational skill building.
- Records accurate and objective data regarding consumer behavior and skills on training programs. Maintains complete, accurate, and current charts and records.
- Practices intervention techniques for consumer falls and acts or incidents of aggression; assists consumers during emergencies; provides first aid when needed.
- Provides after-hours, holiday, and weekend coverage as part of a system of family preservation and family support services as needed.
