Under general supervision, acts as liaison between the CSB and psychiatric facilities as an advocate for persons served and their families involved in the discharge and civil commitment processes. Provides assessment of persons needs and matches them with community resources. Organizes and mobilizes community resources to meet person’s needs. This individual participates as an active member of state facility treatment teams. Monitors persons served and consults on individuals as needed. Provides case management in the community for individuals adjudicated Not Guilty by Reason of Insanity. This position requires obtaining and maintaining state prescreening certification and conducts pre-screening assessments for admission or recertification to psychiatric facilities. Maintains clinical charts and performs other routine administrative functions. Employee must exercise tact and courtesy in contact with staff, persons served families, public, and services providers. This position will work and assist with the Reinvestment Project as directed.
