The position of Medical Support Coordinator is responsible for:
- Functions from a recovery-based philosophy emphasizing hope, choice, and empowerment, personal responsibility, community focus, and
- Organizes and mobilizes community resources to meet the needs of the individual; provides information regarding behavioral and medical issues to improve understanding of needed services.
- Participates in care planning for individual served to link to appropriate services.
- Assesses and identifies the needs of, and resources to benefit, individuals served.
- Clinically documents all activities and coordination efforts in electronic health record (EHR).
- Actively works with medical and other behavioral health professions to coordinate care.
- Ability to effectively communicate with individuals with mental health, substance use/abuse, intellectual and developmental challenges.
- Maintains complete, accurate, and current clinical charts and records; performs routine office filing; receives and answers general correspondence and inquiries.
- Develops and maintains liaisons between all agency programs, other community entities, and organizations offering aid and services to Individuals.
- Educates and contacts community partners/referral sources to coordinate care, relay or gather information, and to build working relationships.
- Coordinates with physicians in the community.
