The position of Crisis Stabilization/Case Manager is  responsible for:

• Provides case management services, as part of a system of family preservation and support services, to those children and adolescents at risk to themselves or others, or at risk of hospitalization or other restrictive placement due to mental illness and/or severe behavioral problems.
• Provides additional case management services, such as coordinating in-patient discharge planning, assisting clients in investigating educational opportunities, living alternatives, and education and entitlement opportunities as relates to family preservation and support services.
• Maintains a cooperative working relationship with agencies collaborating under the state Comprehensive Services Act, including juvenile court services units, departments of social services, schools, public health, and other public and private agencies and individuals involved with children at risk as defined by the Act.
• Provides after-hours, holiday, and weekend coverage as part of the system of family preservation and family support services. 
• Crisis stabilization services to include but not limited to: symptom and behavior management (if you are licensed or licensed eligible, then you can provide short-term counseling; individual/family counseling), psychiatric assessment, compliance with medication management, and treatment planning.
• Support the family during the presenting crisis in increasing safety, provide the child and family with tools/strategies to extinguish the current crisis and reduce the risk of future crises, lay the groundwork for change; help family use the crisis as an opportunity for change; assist the family in becoming its own problem-solving mechanism, and link families to ongoing services.
• Help identify who is in charge, identify family supports and mobilize them, create behavior plans to include having clear rules, consequences, and rewards, create structure and predictability-routines and schedules.
• Teach/Introduce coping skills and emotional regulation to include but not limited to: use of feelings charts, have child and family track emotions daily, use of imagery  , deep breathing, color mandalas, ask for a time out (to be given space), listen to music, exercise, call a support person, etc.
• Support family development to include but not limited to: helping parents monitor for safety-regular safety checks, mirroring how to respond with encouragement, praising the small things, helping parents identify ways to comfort and soothe the child, normalize behaviors and challenges, and link to outside supports that will encourage family growth and decrease risk of future crisis. Example:  YMCA, Therapist, Respite, DSS, etc.
• Maintain complete, accurate, and current clinical charts and records; performs routine office filing; receives and answers general correspondence and inquiries.
• Provide service to persons on caseload through interventions that support the ISP, education/counseling, enhancing community integration, linkages, coordination, monitoring, and other activities.
• Hours will primarily be afternoon/evening, Monday through Friday. Employee will be responsible for covering crisis stabilization services as well as providing crisis case management to consumers. During the summer and when individuals are having breaks from school hours could include daytime hours. Work hours will be in the office, remote, and in the individual’s home providing direct service
• Performs additional case management services on an as needed basis.
• Transports clients to and from assigned locations as requested.
