Job Description
This full-time position involves working directly with individuals in our care, providing personalized support and assistance with daily living tasks. Your duties will include:
- Coordinating schedules for community activities and appointments
- Providing transportation for outings and errands
- Acting as advocate for individuals in the community
- Seeking out community resources to support individuals' goals and objectives
- Recording accurate log entries for documentation purposes
As a key member of our team, you'll participate in the development of Individual Support Plans and contribute to creating a safe and supportive environment for all individuals.
Requirements
To succeed in this role, you'll need a valid Virginia Driver's License, a background check, and certification in CPR, First Aid, Medication Management, and Medicaid Waiver DSP Competencies and Home Based Services.
