The position of Intensive Care Coordinator is responsible for:
- Provides service to persons on caseload through assistance, linkages, coordination, advocacy, monitoring, and other activities in order to prevent out of home placement.
- Organizes and mobilizes community resources to meet the needs of children and adolescents who are in residential; ICC will facilitate a transition back to the community at the earliest and safest appropriate time.
- Performs comprehensive assessments to identify needs of, and resources to benefit, persons served.
- Works closely with family to develop and implement those services necessary to maintain/return child into the community.
- Serves people and their families on assigned case load. Issues present will include any/all of the following: mental health, substance use/abuse, intellectual and developmental challenges.
- Maintains complete, accurate, and current clinical charts and records; performs routine office filing; receives and answers general correspondence and inquiries.
- Develops and maintains liaisons between the FAPT, CPMT, CSB and other community entities and organizations offering aid and services to clients.
- Will be required to transport persons served in the MPNN BH vehicle
BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and 1-2 years’ experience desired working with children and adolescents, emergency/crisis services family counseling, family preservation or case management; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities as outlined in the guidelines set forth by the State Executive Council.
Must possess valid Virginia Driver's License; CPR/First Aid Certifications, Medication Administration Certification, TOVA. Must be certified tuberculosis free 30 days from hire and annually.
