Position Summary
A registered nurse or registered dietitian who is responsible for the planning implementing, monitoring and evaluation of diabetes education and MNT services to individuals, based on an assessment of the patient's needs and the identified goals of the physician in accordance with the educational standards of the American Diabetes Association. The diabetes educator will educate and monitor the patient and family to ascertain whether self-management services provided are adequate and appropriate.
Minimum Qualifications
- Current active unrestricted license to practice in the State of Connecticut as a registered nurse or registered dietitian. NCBDE certification or eligibility for certification as a CDE is required.
- Self-motivated.
- Must possess and demonstrate excellent written and verbal communication skills.
