Position Summary
Ensures comprehensive and thoughtful connection to clinical and non-clinical supports for patients seeking outpatient, mental health care. Engages and gathers clinical histories both for in-person and virtually. Coordinates appropriate access and throughput for patient care.
Essential Duties & Responsibilities
Conducts clinical intakes for patients seeking outpatient mental health treatment
Coordinates the assignment of cases and triage care accordingly
Ensures compliance with related federal, state and local regulations, as well as departmental policies and procedures.
Assists clinicians with throughput, ensuring quality patient care
May conduct/facilitate staff meetings and assist with providing training.
Maintains a small caseload or handles some of the more complex cases; performs a variety of direct clinical, diagnostic, treatment, and outreach services.
Participates with leadership to provide communication of clinical issues related to patient placement/flow
Refers patients to appropriate resources if CBH is unable to meet their needs
Achieves seamless delivery of services by appropriately involving colleagues and physicians to ensure commitment, communication, and cross-functional linkage
Implements appropriate screening tools to efficiently and effectively diagnose and create measurement-based care plans
Partners with clinical leadership to define, reduce, and eliminate processes that may negatively affect patient throughput/efficient delivery of care and implement improvements.
Maintain timely documentation in the electronic health record
Provides consultation to student interns and non-Behavioral Health Care staff regarding Behavioral Health Care services, programs, and guidelines.
Participates in community outreach and public relations activities.

Minimum Qualifications
Master’s degree in social work
Possession of valid clinical license in Connecticut
Ability to diagnose using DSM 5
Must have excellent writing and communication skills, be highly organized
Must be able to work as a part of a team
Preferred Qualifications:
Previous experience involving lead or supervisory responsibility is desirable.
Additional certifications in specialized clinical care such as DBT, CBT, trauma informed care, etc.
Experience/knowledge of substance use disorders, autism, LGBTQ
