Position Summary
The Oncology Nurse Navigator (ONN) utilizes a collaborative process to assess, plan, facilitate, implement and monitor patients along the Cancer Center clinical pathways. The ONN works closely with the patient and family; makes appropriate referrals to meet the individual, cultural and spiritual needs, monitors and evaluates services provided. The ONN works as a liaison to and in collaboration with patients, affiliated physicians, disease management liaisons, and the Middlesex Health System. The ONN also serves as a resource to primary care and specialty physicians in identifying and managing high cost and high-risk patients. The ONN works collaboratively with community venues promoting prevention and early detection via educational programs and screenings.
Essential Duties & Responsibilities
Under general supervision the Lung Nurse Navigator will;
Provides clinical and programmatic support and acts as a resource to TLCC teamCommunicates and collaborates with all departments to optimize operations maintaining high quality and safety of our patients and staff
Attends all departmental meetings and assist manager with identified TLCC issues
Assist with implementation of changes or new processes, auditing roll outs related to the TLCC
Demonstrates support of management mandates to improve patient care and outcomes
Leads all TLCC program quarterly meetings
Works interdisciplinary team to develop and implement pathways in the TLCC
Collecting data related to the quality of the TLCC Program, including data on compliance with program policies and procedures
Works closely with the patient and family; makes appropriate referrals to meet the individual, cultural and spiritual needs, monitors and evaluates services provided.
Works as a liaison to and in collaboration with patients, affiliated physicians, disease management liaisons, and the Middlesex Health System.
Serves as a resource to primary care and specialty physicians in identifying and managing high-risk patients.
Identifies, educates and guides the patient with a cancer diagnosis/abnormal finding/complex urology diagnosis through the Cancer Center and Incontinence clinical pathways in collaboration with the physicians and members of the healthcare team.
Carries a select patient case load as identified by referring physician, clinical staff or direct self-referral.
Navigates and supports each patient through diagnosis and treatment by reinforcing education and coordinating information and care with a team of physicians and health care professionals.
Works collaboratively with community venues promoting prevention and early detection via educational programs and screenings.
Upholds both the Connecticut Nurse Practice Act and American Nurses Association Scope and Standards of professional practice. (Only applies to nursing roles)

Minimum Qualifications
- 3 years RN experience in an acute care facility
- Bachelor’s degree in Nursing
- Registered Nurse licensed to practice in the State of Connecticut
- Independent, self-directed and highly motivated
- 1-2 years Oncology experience
- Lung Cancer experience
- Oncology Nursing Certification (OCN)
