Role: The Stabilization Case Manager's responsibilities encompass all aspects of case management, proper file compliance, adherence to applicable rules/regulations, ongoing evaluation/assessment, crisis intervention, housing-based dispute resolution, ongoing goal plan development and monitoring, referral services, housing retention, placement, and advocacy. Responsibilities will also include monthly unit inspections of Emergency Shelter units and completion of associated documentation and maintenance requests.
Responsibilities:
· Provide stabilization services for a minimum of 30 families for 24 months post-shelter exit
· Accurately complete required documentation in a timely manner and on an ongoing basis
· Maintain and consistently update Client case files, in compliance with program policies/procedures
· Utilize the Efforts to Outcomes (ETO) program to record all case notes, client assessments, touchpoints, demographic information and other functions, as needed
· Ensure that all ETO data is up-to-date and accurate at all times
· Create and enforce Stabilization plans and provide progress updates every 30 days, which includes goal-setting and monitoring of reasonable timelines for completion
· Maintain regular contact with Stabilization Clients through office visits, email/phone communication, and field visits
· Monitor school attendance and educational progress for each school-age child in the family
· Ensure successful transfer of services for families placed outside of the Stabilization program's catchment area
· Maintain monthly communication with Stabilization Client landlords to confirm that the family is paying rent and to identify and potential issues
· Negotiate with landlords and provide dispute resolution between Stabilization Clients and landlords
· Work with Stabilization clients to address and respond to issues raised by landlords, community members, and professional service providers
· Assist Stabilization Clients in accessing and utilizing any public or private resources to support housing retention, as needed
· Exercise knowledge of Agency and program policies/procedures (including applicable policies/procedures issued by DHCD, MBHP, local housing authorities and/or other funders and program administrators)
· Ensure Client compliance with applicable policies/procedures and inform supervisor(s) of a Client's failure to comply
· Maintain and develop referral sources and support networks
· Prepare various types of disciplinary documentation, including Non-Compliances, Serious Incident Reports, Internal Warnings and warning letters.
· Advocate for Clients in areas of need, including health, housing, education, counseling and others
· Attend important appointments with Stabilization Clients as needed, including housing court hearings and landlord mediation.
· Provide crisis intervention, including non-physical de-escalation, emergency troubleshooting and problem solving, reporting to proper authorities
· Schedule and participate in case conferences with clients, supervisors, professional service providers and/or other appropriate individuals
· Supervise Emergency Shelter Client move-ins and move-outs, as needed, and complete necessary unit condition, move-out and/or transfer paperwork
· Conduct monthly unit inspections for a minimum of 25 Emergency Shelter scattered site units
· Ensure Emergency Shelter unit compliance by documenting the findings of all room/unit checks and reporting any issues to supervisors
· Submit maintenance requests for any exterminations, repairs, or other deficiencies identified during Emergency Shelter unit checks
· Assist in the completion of verbal and written 51-A reports for suspected child abuse and/or neglect
· Immediately inform supervisors of any suspicions, observations or allegations of child abuse and/or neglect
· Accurately complete timely incident reports and notify supervisor(s) in the event of emergencies, in accordance with the chain of command
· Abide by all MHSA policies regarding boundaries, strict confidentiality and ethical standards at all times
· Be present and punctual for work
· Attend in-service and outside training and instructions as directed by APD and/or PD
· Various other responsibilities, as assigned by APD and/or PD, from time to time and/or as needed to ensure proper functioning of the Program
