What are the responsibilities and job description for the Medical Discharge Planner position at Middlesex Sheriff’s Office?
The Middlesex Sheriff’s Office (“MSO”) is seeking qualified applicants for the position of Medical Discharge Planner to work in the Program Services and Health Services Department at the Middlesex Jail and House of Correction located in Billerica, MA. The Medical Discharge Planner (hereinafter “Planner”) is primarily responsible for assisting with care by coordinating and supporting access to ongoing medical and mental health services for individuals after incarceration to promote their long-term well-being in the community. The hours of work will be 8:00A.M. to 4:00P.M., Monday through Friday. The hours and days off are subject to change based on the discretion of management and the needs of the Health Services Department. This is an exempt, full time, at-will employment position that reports to the Assistant Superintendent of Program Services and Health Services Administrator or designee.
Additional responsibilities of the Planner include, but are not limited to, the following:
- Scheduling medical and mental health appointments for patients reentering the community;
- Serving as the liaison between Case Managers and medical staff;
- Working with nursing staff to ensure patients medications are ordered for individuals set to release;
- Ensuring medications are at intake before individuals are released or are transported on a court order, when applicable;
- Working with the MassHealth Liaison to ensure that individuals have the appropriate Managed Care Plan;
- Serving as the Certified Application Counselor for MassHealth Applications and assists in establishing relationships and contacts at outside hospitals;
- Working with external partners and medical staff to send bridge scripts to pharmacies for individuals released without medication with verified need;
- Meeting with and providing counsel to individuals around options for medical providers in their release area;
- Fostering partnerships with medical providers to streamline the process of scheduling appointments and providing options for individuals being released;
- Facilitating transitions from sections to medical facilities and back into the community, when applicable;
- Performing MassHealth assessments, when necessary;
- Ensuring all planning and orders are documented in the patients’ medical chart;
- Ensuring all documents are completed for National Commission on Correctional Health Care, Commonwealth of Massachusetts Department of Correction, American Correctional Association and MSO standards;
- Ensuring all data is collected for tracking purposes;
- Performing other duties as assigned by management.
Working Conditions: This position requires:
- Direct contact with inmates/detainees in a secure correctional setting;
- Ability to work varied shifts, weekends, nights, holidays and subject to being forced to work over 8 hours depending on the needs of the Health Services Department;
- Standing and walking for prolonged periods of time;
- May be required to work alone in isolated areas;
- Possible exposure to adverse weather conditions;
- Possible need to lift and/or carry heavy objects up to 100 pounds.
- Must have valid Driver’s License.
The Medical Discharge Planner may encounter verbal and physical abuse from offenders while working in a correctional facility and will be required to interact with people who are under physical and/or emotional stress. The Medical Discharge Planner must also be cognizant of the confidential and sensitive nature of working in a law enforcement agency and is required to comply with all institutional rules regarding safety and security.
Qualifications:
- Bachelor’s Degree preferred.
Preferred Experience, Certifications and Licenses:
- Application Counselor Certification, which may be obtained after hiring.
Additional Qualifications:
- Sound knowledge of all safety protocols in this field;
- Proficiency with computers and related software;
- Excellent communication and customer service skills;
- Ability to work independently and collaborate with internal departments in a fast-paced environment, with a strong focus on attention to detail and a willingness to learn new skills;
- Strong organizational, writing, analytical/problem solving, research and time-management skills;
- Ability to exercise good judgment;
- Ability to attend any other training as required by the department;
- Ability to complete multiple projects in a timely and accurate manner while maintaining comprehensive and cohesive records.
