Family Medicine Physician

Middleton Family Medicine is seeking a passionate, motivated Family Medicine Physician who will work hard to ensure our patients receive exceptional medical care at our growing private Family Medicine practice located in Middleton, MA. This is a critical position that requires a passion to make patient lives meaningfully better. Selected candidates will work in a collaborative team practice that recognizes the patient as the central focus and serves as a patient educator in areas of specialization. This Family Medicine Physician will be expected to assist in the supervision of our mid-level providers, including signing off on clinical encounter documentation. Must be able to assess, diagnose and treat complex illnesses. Other duties include, but are not limited to, providing telephone evaluations and advice to patients, monitoring and managing chronic disease cases of moderate complexity, participating in department initiatives, and serving as the physician on-call for the practice in a rotating schedule with our other physicians.

This position requires a candidate with outstanding critical thinking skills, excellent verbal and written communication skills, computer and EMR proficiency, the ability to work both independently and as part of a team, and a commitment to doing what's best for our patients.

Essential Job Duties and Responsibilities :

Performs new patient screenings, conducts yearly physical examinations, obtains patient medical histories, confirms and updates patients' problem lists

Updates patient medical records and confirms accuracy of information included in the chart at each patient appointment

Orders, interprets, and evaluates diagnostic testing to identify and assess patient clinical problems and healthcare needs

Reviews lab, x-ray, and ED or Admission reports and initiates appropriate patient follow-up to ensure the best long-term health outcomes

Prescribes medications and other forms of treatment such as physical therapy, respiratory therapy, and occupational therapy to appropriately treat patients' acute and chronic conditions and improve quality of life

Oversees care provided by mid-level providers under their supervision and signs off on clinical encounter documentation when appropriate.

Treats acute illnesses and diseases such as cold, flu, and infections

Treats and manages chronic diseases such as high blood pressure and diabetes, with coordination of specialty teams when indicated

Develops and implements treatment plan to address acute and chronic conditions

Provides well child care, including immunizations, sports physicals, and other screenings.

Conducts gynecological exams and pap smears

Cooperates and collaborates with leadership team to implement operational initiatives that drive improvements in patient care and practice performance

Educates patients about health risks, preventive care, diet, exercise, and overall health maintenance

Collaborates with other healthcare providers and professionals in coordination of care and case management

Performs all other related duties as assigned

Education and Licensure Requirements :

MD or DO degree from an accredited school

Massachusetts Board of Registration Licensure

DEA and Massachusetts Controlled Substances Licensure

Key Competencies and Skills :

Computer, technology, and EMR proficiency.

Experience and ability to work with diverse patient populations

Independent worker / Self-starter

Understanding and experience in Ambulatory Medical Care Management

Competency in basic medical and surgical procedures
