Join our dedicated team at Middletown Park Rehabilitation & Healthcare, a renowned 5 STAR rated skilled nursing facility with a proud history of providing outstanding care for over 30 years. We specialize in short-term rehabilitation, subacute, and long-term healthcare services, committed to delivering high-quality care with respect and compassion through a collaborative team approach.
We currently have openings for full-time and part-time Licensed Practical Nurses (LPN), with every other weekend required, and limited per diem opportunities available.

Your Role :
Assist unit leadership by providing timely support and meeting resident and family needs within your scope of practice.
Utilize your nursing skills and judgment to set priorities and ensure quality direct care for residents.
Administer nursing treatments and medications, while closely monitoring residents for both positive and negative effects.
Collaborate with the care team to update residents’ care plans based on observations, lab results, and other essential information.
Document vital signs, treatments, and residents' responses accurately and promptly.
Supervise and mentor CNAs to ensure effective delivery of care, aligning with each resident's needs.
Qualifications :
Graduated from an accredited nursing school.
Active and in good standing LPN license in New York.
Commitment to delivering exceptional, compassionate, and medically sound care.
Ability to foster a cooperative and positive atmosphere that supports resident health and rehabilitation.
Strong interest in caring for diverse populations, including those with complex needs.
Excellent communication, customer service, and organizational skills.
Proven ability to work effectively under pressure and in harmony with fellow staff members.
