Care Manager/ADRC Specialist
Full-Time position
Midland Area Agency on Aging is one of 667 Area Agencies on Aging nationwide and one of eight in Nebraska that help ensure older Nebraskans remain independent, healthy and safe in their homes and communities.
Midland Area Agency on Aging is looking for a full-time Aging and Disability Resource Specialist. This position will be working in our Aging & Disability Resource Center.
Primary Responsibilities:
· Perform screening & assessment of clients.
· Work with the general public, particularly adults who are elderly or disabled
· Considerable knowledge of social problems, such as family disorganization, domestic violence, abuse and neglect, discrimination, disability, poverty, unemployment, aging, and their effect on the individual.
· Considerable knowledge of community resources and services as they pertain to developmental disabilities, mental health, physical disabilities, and the elderly
· Ability to develop and maintain effective working relationships with clients, families, multi-disciplinary staff, caregivers, service providers, volunteers, community agencies, and the public.
· Ability to participate in multi-disciplinary decision making and services coordination
· Assist in connection individuals to programs, services and public benefits
· Ability to interpret and explain programs and services available to clients and their families. Ability to communicate effectively both orally and in writing. Ability to effectively interact with sensitivity with persons from diverse cultural, socioeconomic, educational, racial, ethnic, and professional backgrounds, and persons of all ages and lifestyles
Required Qualifications:
- Associates Degree, Bachelors Degree preferred
-At least an equivalency of two years of related, professional experience; paid or unpaid; (employment; college internships; volunteering at philanthropic, community and social organizations) in long-term care, gerontology or community health.

Candidates will receive credit for all qualified experience.
- Bilingual (fluent) in English and Spanish preferred.
· Current Driver’s License
· Experience working with MS Word, Excel, and Outlook
· Ability to travel in assigned region to visit members in their homes
· Case Management experience
· Behavioral health experience
· Field based work experience
· Experience in long-term care, home health, hospice, public health, or assisted living
· Knowledge of community resources
This position requires travel to clients home in the assigned service area.
