Summary:
The Home Care Coordinator will be responsible for identifying and developing a plan with the participant and/or caregiver to assist in the coordination of in-home services. The plan will outline expectations, supports, quantity, and confirmation of in-home care services needed. In addition, duties will include providing a summary/profile of home-based care for each participant and their residence at meetings and participating in transitional planning.
Midland Cares Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Salary starting at $44,000 /year (pay is determined by requirements and experience for this role)
Employment Status: Full-Time | Monday Friday | 8am-5pm

Qualifications
Required: One or more years of healthcare experience working with seniors required. Must possess a valid Kansas Drivers License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organizations insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position.
