Community Care Hospice is now hiring for a Full-time Hospice RN Case Manager for Hutchinson and surrounding area.
Summary
In this role you will have the primary responsibility to participate as a member of the interdisciplinary team to assess, plan, implement, and evaluate care provided in the patients place of residence. You will actively participate in coordination of all aspects of patients care, in accordance with current professional standards and practice. This position will also require flexibility to meet patient care needs within the continuum of services and participate in the Community Care Hospice 24/7 nursing coverage rotation.
Employment Status: Full-time | Monday-Friday
Pay: Starting at $70,000 based on like experience
$3,000 Sign on Bonus!

Qualifications
Required: Current unrestricted State of Kansas License to practice as a Registered Nurse. One year professional medical-surgical experience. Must possess a valid Kansas Drivers License, in force automobile insurance and maintain a Motor Vehicle Record (MVR) that meets the organizations insurability standard as defined by Community Care Hospice policy. Availability of reliable transportation is required. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible.
Preferred: Home Health or Hospice experience preferred.
