We are seeking a compassionate and detail-oriented Intake LPN to support our Home Health intake and referral process. This role involves coordinating with referral sources, evaluating patient eligibility, and ensuring a smooth intake process.
Key Responsibilities:
- Communicate with referral sources to facilitate a timely intake process.
- Evaluate patient eligibility and assist with insurance verification.
- Provide administrative support to clinical staff and field teams.
- Ensure accurate documentation in electronic medical records.
- Participate in performance improvement and professional development activities.
Pay: Starting at $27.30/hr. based one experience
Full-time: Monday-Friday | 8am-5pm

Qualifications:
- Active LPN License (State of Kansas and Missouri)
