Summary:
Midland Care is seeking a RN Case Manager to oversee and coordinate the care of our PACE Participants. In this role you will assess participant needs, develop tailored care plans, monitor progress, and collaborate with healthcare professionals to promote optimal outcomes.
Midland Cares Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Employment Status: Full-time | Monday-Friday
$3,000 Sign-On Bonus **
Pay starting $67,000 /annually (pay is determined by requirements and like experience)

Qualifications
Required: Current unrestricted State of Kansas License to practice as a Registered Nurse. Availability of reliable transportation is required. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible. Must possess a valid Kansas Drivers License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organizations insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position.
Preferred: Home Health, PACE, or Hospice experience.
