The Community Care Coordinator (CCC) is involved in assisting with outreach and coordination with all Permian Basin services that are Juvenile Justice Stakeholders. The CCC assists the department in monitoring the provision and quality of services provided to the family and is the family liaison when new services / resources need to be sought or developed. This staff serves as the representative for the department for referrals and referral processes to community agencies that are providing preventative / intervention / rehabilitative services or any type of support for juveniles of Midland County. The CCC is a member of the program team that works to identify community resources available to meet current department needs for prevention / intervention / rehabilitation.
Essential Duties and Responsibilities
- Outreach and coordination with all community agencies in Midland County that provide services and / or support to juveniles in the area of prevention / intervention / rehabilitation.
Assist JPO's with referrals to community agencies for preventative / intervention / rehabilitative services or any type of support for juveniles.
Works in collaboration with Programs, JPO's and outside community agencies to assist in securing services to meet identified needs and brokers both traditional and non-traditional services individualized to meet family needs.
Assists to identify service gaps and / or lack of community resources.
Serves as department representative to the Community Resource Coordination Group (CRCG) meetings.
Knowledge of Wraparound Services principles and processes.
Serves as JPD Liaison in the community to identify resources locally.
Serves as department point of contact for community program / service providers.
Facilitates all JMT (Juvenile Medicaid Tracker) referrals from the department.
Provide recommendations to Deputy CJPO Programs regarding community services for juveniles.
Provide support, guidance, and advocacy for youth, when appropriate, in the school setting, legal setting, and in the community.
Assist with the development and implementation of curriculum-based programs for youth.
Maintain regular contact with families; model and reinforce appropriate social interactions for them.
Assist JPO's with field visits and placement visits.
Assist JPO's with transport for youth to appointments and any services in / out of Midland County.
Willing and able to work non-traditional hours when needed.
Perform all other related duties and responsibilities as assigned for the purpose of ensuring the efficient and effective functioning of the department.
Performs other job-related duties as assigned.
Qualifications, Education, and / or experience
QUALIFICATIONS

EDUCATION and / or EXPERIENCE
Must be over the age of 21, a U.S. Citizen, have valid Texas driver's license, pass a criminal background check.
Requires bachelor's degree plus at least one year of experience working with juveniles, or one year of graduate study or any equivalent combination of education and experience that provides the required knowledge, skills, and abilities
OR High School Diploma and five years of experience of wraparound principles / processes, family partnering and / or youth advocacy.
Knowledge, Skills and / or ability
KNOWLEDGE OF
Juvenile justice system; Texas Family Code, juvenile law; all TJJD standards, and standard office practices and procedures.
SKILL / ABILITY TO
Operate computers; read & write reports, letters, and other documents using proper format and grammar. Communicate effectively, both orally and in writing; physically restrain other individuals; and establish and maintain effective working relationships with co-workers, juveniles and their families, and the general public.
HEALTH / DENTAL INSURANCE- Regular full-time employees are provided general medical, hospitalization, and dental insurance coverage. This is free to the employee if they take a yearly HRA and have negative Cotinine test yearly. Employees may obtain coverage under the group plan for family members at their own expense. Employees become eligible for this coverage on the 91st day of employment. Prices for other insurance are listed
LONG TERM DISABILITY- Regular full-time employees working over 30 hours per week are eligible for long-term disability benefits. This benefit becomes effective on the 1st day of the month following six months of continuous employment with the county
CAFETERIA PLAN - All regular full-time employees are eligible to enroll in the supplemental insurance portion of the county cafeteria plan on the 91st day of employment, or annually during an enrollment period for the following plan year of January 1 through December 31. They will not be eligible for enrollment in the Flexible Spending Accounts for medical reimbursement and / or dependent care until the first annual enrollment for the plan year beginning after 1 full year of employment.
DEFERRED COMPENSATION PLANS - Midland County offers five deferred compensation plans. Enrollment in one of these plans can be done at any time after the 91st day of employment.
RETIREMENT PLAN - The county is a member of the Texas County & District Retirement System (TCDRS). Regular employees are required to become members of TCDRS and are eligible for this benefit immediately upon employment. Employees automatically deposit 7% of their salary. Temporary employees are not eligible to become members of TCDRS. The county matches employee deposits by 200% and interest at retirement. An employee is vested after 8 years of service. An employee may retire with full benefits after 8 years of service and attainment of age 60, or after 30 years of service, or at the time the employee qualifies under the "Rule of 75". The "Rule of 75" is defined as when the employee's age combined with years of service (rounded down to whole years) equal to 75 or more.
DEATH BENEFIT PLANS - There are two (2) types of death benefit plans provided by Midland County to all employees who participate in the Texas County and District Retirement System.
SURVIVOR ANNUITY DEATH BENEFIT -To be eligible for this death benefit, the employee must be a participant in T.C.D.R.S. and must have completed at least four (4) years of service with Midland County
SOCIAL SECURITY - All employees of the county are covered by social security. The county also contributes to the social security system on behalf of each employee.
WORKERS' COMPENSATION - All employees of the county are covered by the workers' compensation insurance program, and the county pays the premium. This coverage provides medical and salary continuation payments to employees who receive bona fide, on-the-job, work-related injuries.
LEAVE TIME - Regular full-time county employees are eligible for holidays, vacation leave, sick leave, and other types of released time under certain circumstances.
FREE WELLNESS CLINIC- for anyone over the age 3 covered by Midland County's insurance.
