Job Description:
The Registered Nurse (RN) Trauma Case Manager is accountable and responsible for coordinating the care and service of trauma patients with physicians, nurses and other members of the trauma team to facilitate the plan of care. The RN Trauma Case Manager is also responsible for ensuring that the patient is placed in the appropriate level of care, monitoring the utilization of healthcare resources.

SHIFT AND SCHEDULE
Monday - Friday: 8:00 AM - 5:00 PM
ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS
- Collects in-depth information about a patient’s situation and functioning to identify those individuals’ needs and develop a comprehensive case management plan that will address those needs.
- Executes specific case management activities and/or interventions that will lead to accomplishing the goals set forth in the case management plan.
- Conducts concurrent trauma reviews including performance improvement.
- Documents the patient’s progress on a daily basis and identifies and addresses issues in a timely and efficient manner.
- Responds to trauma activations while in house and serves as a resource person regarding the case management process and the care of the trauma patient.
EDUCATION AND EXPERIENCE
- Bachelor of Science (BSN) Degree in Nursing required.
- Current State of Texas License as a Registered Nurse
- Certification in area of specialty (e.g., CEN, CPEN, CCRN, TNS) preferred
- ACLS, PALS, ENPC and TNCC required within 18 months of date of employment.
- Four years Emergency Department or Critical Care experience preferred.

- Stand, walk, sit, stoop, reach, lift, see, speak and hear. Lifting is limited to 35 lbs. for clinical staff and to 50 lbs. for non-clinical staff. The individual must use an assisted-lift device or get another individual(s) to assist with the lift that is over these maximum limits.
