Key Responsibilities:
- Perform comprehensive nursing assessments and implement individualized care plans tailored to resident needs.
- Administer medications and treatments per physician orders, monitoring for therapeutic responses and adverse reactions.
- Ensure accurate and timely documentation of assessments, interventions, and outcomes within the electronic health record (EHR) system.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and modify care plans for optimal patient recovery.
- Supervise and support Certified Nursing Assistants (CNAs) and other nursing personnel, ensuring best practices in resident care.
- Provide patient and family education on medications, disease management, and preventative health strategies.
- Maintain compliance with state, federal, and facility guidelines while upholding the highest standards of infection control and patient safety.
What We’re Looking For:
- Active Texas LVN or RN license in good standing.
- Minimum 1 year of clinical experience in skilled nursing, post-acute care, or rehabilitation (preferred).
- Proficiency in clinical assessment, critical thinking, and evidence-based nursing practices.
- Strong communication and interpersonal skills, fostering team collaboration and compassionate patient interactions.
- Experience with Electronic Health Records (EHR) and ability to document accurately and efficiently.
- Current Basic Life Support (BLS) certification.
