We are looking for a Full-time, 40 hours/week, IRIS Consultant to work (hybrid) remotely to serve Lincoln, Langlade, and Oneida Counties in WI.
Monday through Friday, 9 a.m.-5 p.m.
Will be required to attend face-to-face team meetings with other ICs and their supervisor in the Stevens Point office. In addition, IC and supervisor phone check-ins may occur.
Under the general supervision of the IRIS Consultant Agency’s (ICA) Self Directed Services Program Coordinator the IRIS Consultant is responsible for providing ongoing support and assistance to participants enrolled in the IRIS program within the ICA’s service area.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
DUTIES include but not limited to:
Provide orientation to new participants of the IRIS program
Inform participants of their rights and responsibilities as a participant in the IRIS program
Act as the primary contact for the IRIS program to participants
Assist participants to complete necessary paperwork, answer questions and address concerns regarding the IRIS program
Schedule and complete annual redetermination LTC-FS within 365 days of the previous screen or whenever a change occurs in participant’s condition
Collaborate on the development, process improvement and ongoing quality management of the services provided
Assist participants in identifying their long-term support needs, desired goals, and outcomes
Assist participants in identifying informal or natural supports to help meet their desired outcomes
Assist participants in the development and implementation of an Individual Services and Supports Plan (ISSP)
Collaborate with participants, natural supports, service providers, community resources and others to assist participants to meet their identified outcomes
Accurately complete and submit to DHS all required forms, assessments and documentation for review and approval as indicated by the IRIS program policies and procedures
Assure program quality through various quality management activities that are carried out in partnership with the participant and DHS to include but not limited to personal experience outcomes, provider qualifications, participant emergency and back-up plan, critical incident reporting and service plan and budget monitoring
Meet regularly to ensure participant’s outcomes are being met while staying within their IRIS budget and within the requirements of the IRIS program determined by DHS
Maintain positive working relationship with ICA staff, outside agencies, and service providers to ensure effectiveness of services provided
Attend all ICA and DHS trainings, conferences and meetings as requested and required
Adhere to and abide by all ICA policies and procedures.
Additional Duties Based on Qualifications/Education:
Complete an objective, in person Adult LTC-FS with IRIS participants as required and outlined in the Wisconsin Adult LTC-FS Instructions
Achieve a passing score on the Continuing Skills Testing (CST) conducted every two (2) years to maintain certification
Meet screening quota
QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill, and/or ability required.
18 years of age or older
Bachelor’s degree desired in social work, psychology, human services, counseling, nursing, special education, or a closely related field AND have one (1) year of supervised experience working with seniors and/or people living with disabilities OR have a minimum of four (4) years of direct experience related to the delivery of social services to seniors and/or people living with disabilities and long-term care needs in a community setting.
REQUIREMENTS:
Completion of all required IRIS orientation and training courses required
Understanding of Include, Respect, I Self-Direct philosophy and the principles of self-direction, responsible financial management, and person-centered planning processes required
Understanding of ICA services including behavior management, risk and needs assessments, service plan development and identification and reporting of critical incidents of abuse, neglect, exploitation, and fraud required
If applicable, certified as a LTC FS Screener through completion of the DHS web-based clinical certification required
Working knowledge of Microsoft Office, Outlook email and internet required
Competence in applicable areas of the Wisconsin Self-Directed Information Technology System (WISITS) and DHS SharePoint sites required
Knowledge of community resources required
Strong customer service skills required
Good organizational and time management skills required
Ability to travel within the ICA service area and statewide required
Possess a valid Wisconsin driver’s license, provide proof of auto liability insurance meeting agency minimum limits and be able to meet agency driving standards or an alternate effective means of transportation required
Ability to maintain confidential information including but not limited to financial data, consumer information or other non-public proprietary agency information required
Ability to pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Check required
