What are the responsibilities and job description for the Respite Care Provider position at Midstate Independent Living Choices, Inc (MILC)?

Respite Providers needed in Tomahawk, WI
20 hours per month.
Interested candidates may apply for any or all hours available.
Duties include but not limited to:
Providing companionship
Activities such as game/card playing and walks based on consumer preference
Assistance with bathing
Assistance with toileting including use of bedpan, urinal, commode or toilet
Assistance with dressing and undressing
Assistance with mobility and ambulating including use of walker, cane and crutches
Assistance with simple transfers including bed to chair or wheelchair and reverse
Changing of consumer(s) bed, laundering of bed linens and consumer(s) personal clothing
Skin care (excluding wound care)
Care of eyeglasses and hearing aids
Light cleaning in essential areas of home during personal care activities
Meal preparation and meal serving
Provide errand services and/or transport consumer
Promptly reporting changes in the consumer's condition to the designated supervisor
Completion of all time sheets and documentation necessary to maintain program services in a thorough, accurate, and timely manner
Maintain confidentiality of consumer’s information
Adhere to and abide by all agency policies and procedures
Qualifications:
Familiar with the philosophy and concepts of independent living required
Ability to understand and carry out consumer instructions required
Ability to relate well to others required
Good past employment references required
Physical strength sufficient to perform essential functions of job required
Effective means of transportation to the work site required. If consumer Plan of Care includes errand services or transport of consumer, a valid Wisconsin driver’s license, proof of auto liability insurance meeting agency minimum limits and ability to meet agency driving standards required
Completion of interview and training conducted by PAS staff required
Satisfactory Criminal Background results required. Results based on relevance to position and in compliance with state and federal regulations for personal care workers.
Some positions require ability to transport the consumer and/or provide errand services.
