Job Title: Case Manager
Schedule: Full-time, 36-40 hours per week, Monday-Friday
Rate of Pay Range: $20- $23 per hour Depending on Experience

JOB SUMMARY:
The behavioral health case manager is a core member of the integrated care team, including the patient's medical provider and behavioral health consultant, as well as the larger primary care team. The behavioral health case manager is responsible for coordinating and supporting mental health care within the clinic as well as assisting patients to access clinically indicated resources in the community. The behavioral health case manager will organize and track referred patients, monitor patient progress, address patient needs, and coordinate patient follow-up to ensure continuity of care. The behavioral health case manager will work closely with the behavioral health consultant to develop, track, and monitor patient care plans. The behavioral health case manager will be responsible for proactively engaging patients and functioning as liaison between primary care and behavioral health services. In addition, this position maintains communication channels between Midtown and other community based organizations and programs.
ESSENTIAL FUNCTIONS:
- Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical provider and the behavioral health team.
- Facilitate patient engagement and follow-up care.
- Provide patient education about common mental health and substance abuse disorders and the available treatment options.
- Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Participate in regularly scheduled (usually weekly) caseload consultation with the behavioral health consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
- Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the EHR and use the system to identify and re-engage patients.
- Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, behavioral health consultant, and other treating providers.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the behavioral health consultant and who may need more intensive or more specialized mental health care.
- Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, work force services, vocational rehabilitation, mental health specialty care, substance abuse treatment).
- Assist patients in filling out/completing required documents/forms for community resources/agencies.
- Develop and complete relapse prevention/self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
- Conducts social checks to assess for social determinants of health and coordinates referrals to community programs.
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Maintain effective and professional relationships with patient and other members of the care team.
- Work with patients who have common mental health and/or substance abuse disorders.
- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.
- Work with patients by telephone as well as in person.

JOB QUALIFICATIONS:
Education & Experience:
High Diploma or equivalent required. Bachelor's degree from a four-year college or university in psychology, social work, or related field preferred; 1-2 years of previous experience in linking consumers with various community resources preferred. Experience with assessment and treatment planning for common mental health and/or substance use disorders.
Working knowledge of evidence-based counseling treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
Basic knowledge of psychopharmacology for common mental health disorders.
Communication Skills:
Must possess the ability to communicate verbally and non-verbally with clients, coworkers and members of management. Excellent professional customer service skills required. This position requires the ability to speak Spanish.
Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft Office software and Electronic Medical Records software.
Certificates and Licenses
Must maintain a valid UT driver's license.
