Social Services Director – Midtown Oaks Post Acute
Location: Sacramento, CA
Pay: $30 - $40 per hour (DOE)

Position Overview
The Social Services Director plays a vital role in enhancing the well-being of our residents by providing emotional support, advocacy, and discharge planning. This position ensures compliance with state and federal regulations while working closely with residents, families, and interdisciplinary teams.
Key Responsibilities
Resident & Family Support:
Conduct psychosocial assessments upon admission and update as needed
Provide emotional and social support to residents and families
Address resident concerns and serve as an advocate for their needs
Facilitate resident and family meetings to discuss care plans and discharge goals
Discharge Planning & Community Integration:
Develop and coordinate individualized discharge plans in collaboration with nursing and therapy teams
Connect residents and families with appropriate community resources (home health, hospice, transportation, housing, etc.)
Educate residents and families on post-discharge care needs and available services
Regulatory Compliance & Documentation:
Ensure compliance with federal, state, and facility policies regarding resident rights, social services, and discharge planning
Maintain accurate and timely documentation of social services interactions, progress notes, and care plan updates
Participate in interdisciplinary team meetings, surveys, and audits to ensure best practices in resident care
Collaboration & Program Development:
Work closely with nursing, therapy, and activities teams to provide a holistic approach to resident care
Establish and maintain relationships with hospitals, agencies, and community partners to enhance resources for residents
Assist in implementing programs that promote resident engagement, well-being, and satisfaction
Qualifications & Skills
✅ Experience in social work, psychology, or a related field (SNF experience highly preferred)
✅ Knowledge of discharge planning, case management, and community resources
✅ Strong interpersonal and communication skills to work with residents, families, and teams
✅ Ability to handle sensitive situations with professionalism, empathy, and discretion
✅ Familiarity with state and federal regulations for skilled nursing facilities
