Job Overview:
The Cardio@Home Coordinator manages the Cardio@Home program, including CardioMEMS, Vivify, and other remote patient monitoring systems. This role ensures the optimal care of patients with chronic cardiovascular conditions like Congestive Heart Failure (CHF) and Acute Myocardial Infarction (AMI) through remote monitoring, virtual visits, and coordination between patients, physicians, and hospital partners.
Key Responsibilities:
- Collaborate with clinical staff to manage care for Cardio@Home patients.
- Monitor web-based dashboards to ensure compliance with care protocols.
- Review medical records to assess patient care quality and timeliness.
- Educate staff and provide feedback to improve program compliance.
- Participate in community outreach and business development for the program.
- Assist in presenting data and performance metrics to leadership.
- Perform other duties as assigned.
Qualifications:
- Required:
- Graduate of an accredited nursing school.
- Current licensure with the Illinois State Board of Registered Nurses.
- BLS and ACLS certifications.
- Strong communication and organizational skills.
- Preferred:
- Advanced certification in Cardiovascular Nursing.
- 2 years of critical care experience.
Compensation: Pay starting at $34.3750. Pay will commensurate with experience
