We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Midwest City Post Acute Rehab. Key Responsibilities:
- Provide direct patient care, including administering medications and treatments as prescribed.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Monitor and assess residents' health, promptly addressing any changes in condition.
- Maintain accurate and up-to-date medical records, ensuring compliance with regulatory standards.
- Communicate effectively with residents, families, and team members to provide comprehensive and compassionate care.
- Assist in the coordination of resident activities and therapeutic programs.
- Participate in quality improvement initiatives to enhance resident outcomes.
- Adhere to infection control and safety protocols.
- Provide support and guidance to Certified Nursing Assistants (CNAs) and other team members.
Qualifications:
- Current and valid LPN license in the state of Oklahoma.
- CPR certification.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Experience in a long-term care or rehabilitation setting is preferred but not required.
