Essential Job Duties & Responsibilities
- Evaluate patient and perform assessments; clinical documentation; care plan development; communication with the physician, care team members, patient, and family.
- Wound care, Infusion therapy, and Disease Management.
- Provide direct supervision of nursing clinical care team.
- Administer medications and treatments prescribed by physician.
- Participate in regular case conferences and in-services.

Requirements- Current Wisconsin nursing license
- Negative TB test within last 6 weeks
- Experience with Point Click Care (PCC)
- Experience in wound care
- At least 3 months experience in long-term care
- Competency to carry out procedures that the physician orders that are within the scope of nursing
- Proficient communication skills
- Strong organizational skills and time management skills
- Exceptional customer service
- Working as a team mentality
- Reliable transportation
