Job Summary: The Service Coordinator fulfills all duties of the Designated Coordinator in a Rule 245D program licensed by the MN Department of Human Services. This position provides case management services, direct care and instruction for adults with developmental and other disabilities in a day training and habilitation program. Duties include:
- Working directly with persons in the program throughout the day and providing leadership/work direction to other program staff to ensure that individualized plans are properly implemented and that people served in the program receive quality services.
- Performing all aspects of internal case management, developing individualized program plans, acting as a member of the support team for persons served, acting as the liaison with each participant’s family and residential provider.
- Planning and implementing individualized and small group activities within the center and the community.
- Working closely with the Program Supervisor and Program Director to ensure quality services and adherence to all program and regulatory standards.
Hours, Benefits: 7:30 AM to 4:00 PM, Mon- Fri; full time. No evenings or weekends. Competitive wages. Benefits include health, life and disability insurance, paid holidays and PTO and a 401K pension plan. AA/EEO Employer. MSS provides all training for a commercial driving license as applicable.
Qualifications
Must possess the education and experience necessary to provide quality services which would include: A BA/BS degree in a related field and 1 year work experience providing direct care for persons with disabilities; or an AA degree in field related to human services and 2 years of work experience providing direct care for persons with disabilities; or a diploma in a field related to human services from an accredited postsecondary institution and 3 years of full time work experience providing direct care services to persons with disabilities; or A minimum of 50 hours of education and training related to human services and 4 years of full time work experience providing direct care services to persons with disabilities and older under the supervision of a staff person who meets the qualification identified in the above criteria.
You must also have a valid driver’s license, a clean driving record and be insurable under the standards set by our insurance company. Candidates must pass a drug screen and criminal background screening as determined by DHS and MSS. Must be able to frequently stand, bend, stoop and reach and push manual wheelchairs used by adults.
Note: The ideal candidate must be comfortable providing leadership and working in a team environment Preference will be given to candidates who have experience in art, theatre or music; woodworking, green screen experience or 3D printing and has experience with MN Statues 245D.
