(FT) DIVERSION YOUTH CASE MANAGER
SUMMARY:
The position of Diversion Youth Case Manager will serve as the single point of contact for diversion youth programming for case management. The diversion case manager will provide intake, assessment, case management, advocacy, limited counseling, and referral services to youth referred to Midwest Youth Services in lieu of system involvement. The diversion youth case manager will work alongside the diversion specialist to deliver restorative mentoring programming and facilitate an individualized case plan with youth referred to programming. The diversion youth case manager will maintain a caseload of 20-30 clients at any given time. Primary goals of these programs include: 1) Diversion of youth from child welfare and juvenile justice systems; 2) Preserve or reunify families; 3) Develop a continuum of comprehensive community-based youth services; 4) Close service gaps by identifying and addressing service voids; 5) Coordinate education, employment, training and other programs for youth; and 6) Encourage the use of qualified volunteers and voluntary associations. 7) Assist youth and families in healing, recovery, and growth.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Represent the agency in a positive and professional manner at all times.
2. Interact with Midwest Youth Services, Inc. staff, Law Enforcement (i.e. Sheriff offices, City Police Departments, State’s Attorney’s, Judges, and Probation Offices), School Districts and other referral sources and agencies within the counties of Brown, Cass, Morgan, Scott, and Schuyler. Provide training or information to community partners regarding childhood trauma and its impact, using a universal approach for consumers who may have been exposed to trauma.
3. Participate in the after hours on-call crisis rotation. The Diversion Youth Case Manager will process intakes as presented during regular office hours.
4. Participate in program outreach, awareness, and agency fundraising efforts.
5. Participate in job specific and trauma informed educational training opportunities, seminars, and retreats
6. Participate in facilitator training opportunities as directed, funded, and advised for program growth and service delivery by Midwest Youth Services.
7. Complete comprehensive assessment on each client and adhere to program service delivery within stated timelines.
8. Implement appropriate client service plans that may include but are not limited to: Counseling, advocacy, employment and education assistance, access to drug, alcohol and mental health services and other appropriate referrals
9. Facilitate, conduct, and accompany clients and families to community-based services and linkages. (barrier relief - accessibility, communication, transportation, etc.)
10. Maintain up to date client files on all assigned cases, and complete appropriate paperwork for walk-ins and referrals.
11. Provide ongoing case management to ensure appropriate services are delivered
12. Provide monthly billing information to the business office, and provide information as requested for the completion of other reports
13. Recognize trauma symptoms and demonstrate ability to support healing and recovery. Engage caregivers, parents, family members by sharing strengths and challenges experienced by their children.
14. Demonstrate de-escalation and redirection skills.
15. Complete all other assigned duties as professionally determined

Pay: $42,000.00 - $46,500.00 per year
