POSITION SUMMARY
The House/CCS Lead will work full-time under the supervision of the Service Coordinator and will work with adults with intellectual/developmental disabilities (I/DD), in a residential and/or community setting. The House/CCS Lead will monitor, supervise, and manage the Direct Support Professionals and the support being delivered to ensure the use of best practices within the community and residential setting to support and teach people with intellectual development disabilities how to acquire the physical, emotional, and social skills and knowledge to live as independently as possible. The lead must be able to handle the pressure of training in various environments, exercise patience, work with flexible schedule, and respond to crisis situations. Ingenuity and creativity must be utilized when ensuring the proper implementation of the various supports to individuals in supports as outlined by their individual serve plan. The lead is responsible for assuring that Milagro de Vida programs meet all HCA and DDSD principles and state/federal guidelines.
QUALIFICATIONS:
· High School Diploma or GED equivalent with 2 years’ experience in related field; or a bachelor’s degree with 1 year experience in a related field.
· Valid state driver's license and good driving record.
· Three (3) personal references, two (2) of which will be job or education related.
· Interest in and commitment to serving the needs of the developmentally disabled within the rights afforded to them as citizens.
· Clear record of abuse/neglect/exploitation
· Successful completion of the post-offer process including criminal history check and professional references.
· Successful completion of Milagro de Vida’s Orientation and Management Training, as well as any required state trainings, including individual specific training requirements.
· 1 year management/supervising experience preferred
WORKING CONDITIONS:
· Hours and weekly schedule dictated by current tasks, to include evening and weekend hours as required.
· While performing the duties of the job, employees travel by automobile and is exposed to changing weather conditions.
· May transport persons’ served in their own vehicle.
· May occasionally assist with wheelchair transfers with non-ambulatory persons served.
· May be exposed to various medical conditions and communicable diseases.
· Work will take place in the community and home/residential settings.

ESSENTIAL JOB FUNCTIONS:
· Must be able to complete all required documentation and ensure implementation of all Milagro de Vida policies and procedures, state and federal requirements as well as individual service plans for assigned persons in supports.
· Must be able to arrange, train, and monitor completion of training requirements for assigned Direct Support Professionals.
· Must be available and “On call” 24 hours a day in collaboration with other management staff.
· Must be able to perform physical activities, including lifting up to 50 lbs., transferring individuals in supports, walking, pushing wheelchairs, kneeling, bending, reaching and occasionally remaining on feet for long periods of time.
DUTIES AND RESPONSIBILITIES:
1. Responsible for hiring and supervising Direct Support Professionals (DSPs) to provide residential and day habilitation support to ensure compliance with work objectives.
2. Completing performance reviews and all required Human Resources documentation for assigned DSPs
3. “On call” 24 hours a day in collaboration with other management staff.
4. Distributes and communicates pertinent information to assigned DSPs.
5. Assist Service Coordinator in maintaining and monitoring accurate system for tracking staff schedules for all residential and day habilitation programs to ensure schedules are in accordance with individual’s needs.
6. In collaboration with the Service Coordinator responsible for scheduling and tracking staff schedules, to ensure schedules are in accordance with individual specific needs as they are related to program services.
7. In conjunction with Service Coordinator, develops, coordinates, and monitors community integration and day habilitation schedules for the day program including CLE calendars and schedules and events calendar as scheduled.
8. In concurrence with the Service Coordinator and person’s Interdisciplinary Team (IDT), develop training curriculums and provide training to residential and day habilitation staff to ensure individuals served are actively engaged in meaningful activities.
9. In collaboration with the Service Coordinator, monitor and schedule required staff training’s, seminars, and conferences.
10. Monitors fiscal, medical, personal, and ISP documentation to ensure accuracy.
11. Monitors and ensures house and vehicle maintenance, disseminates instructions as necessary to facilitate repairs.
12. Monitors and tracks health and medical needs of individuals, disseminates instructions necessary to facilitate corrections or implement preventive measures.
13. Builds a support network between the individuals and community members.
14. Monitors and maintains an efficient and accurate system for tracking and ensuring that accurate “in house” documentation is completed and submitted withing established time frames.
15. In collaboration with the Service Coordinator, prepares and submits required written reports to individual’s IDT, Executive Director, and/or other required program entities within established time frames.
16. Ensure that all In-House documentation is accurately completed for each location and submitted as per established time frames, in accordance with Billing Guidelines.
17. Ensure consistent service delivery that connects work, home, and recreational needs.
18. Ensure that all support plans and requirements, as outlined in the individual service plans, are followed by Direct Support Professionals to ensure consistent service delivery.
19. In collaboration with the Service Coordinator, participates as an active member of each person’s IDT, to include the development of the ISPs.
20. May perform the duties of DSPs as necessary, in accordance with current staffing needs.
21. Attends conferences, seminars, and training as assigned by the Executive Director and/or Service Coordinator.
22. Other duties as assigned by the Executive Director and/or Service Coordinator.
