Job Overview:
THIS IS AN IN PERSON POSITION:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and coordination of care for patients in an outpatient and community setting. Your role will involve collaborating with other professionals, patients, and their families to ensure the best possible outcomes.
We are looking for Strengths Based Case Managers to work with our outpatient clients. This position may be part or full time hourly or contractual depending on applicant’s availability. We have opportunity for those who are early in their education still, already have obtained an associates or bachelors in social work and who either have or are willing to earn a CADC developmental plan to work with those with substance use disorders. This is an excellent position for those in school as you will be supervised by a LMSW and a CCS from MCBAP. This job is for a self-starters and have good time management, excellent paperwork skills, therapy type skills, and assertiveness including the strong use of boundaries and support of staff. We have a team approach that requires constant communication with both supervisors and other team members. You must be quick on your feet and able to manage a crisis for this job.
Duties:
- Conduct assessments to determine patient needs and develop individualized care plans
- Monitor patient progress and adjust care plans as needed
- Provide mental health counseling and support to patients experiencing emotional or psychological challenges
- Collaborate with social workers, managed care organizations, and other entities to coordinate services and resources for patients
- Assist with discharge planning to ensure a smooth transitions
- Maintain accurate and up-to-date documentation of patient interactions and progress
Skills:
- Strong knowledge of HIPAA regulations and ability to maintain patient confidentiality
- Excellent interpersonal skills with the ability to build rapport with patients, families, and healthcare professionals
- Strong organizational skills with the ability to prioritize tasks and manage multiple cases simultaneously
- Effective communication skills, both verbal and written
- Willingness to enroll in a MCBAP Development Plan.
