Position: Clinical Care Coordinator/Transitions Program (Long-Term Substitute)

Schedule: January 31, 2025 – February 28, 2025

Qualifications:
•Master’s degree in Social Work, Counseling Psychology or related field required
•LICSW or LMHC required
•Experience working with youth in a school and clinical setting
•Enthusiastic and student centered; committed to high standards for all students
•Excellent communication skills and ability to collaborate with administrators and crisis teams
•Strong diagnostic skills
•Knowledge of evidence-based treatment
•Bilingual in Spanish or Portuguese preferred but not required
Job Responsibilities:
•Provide short term assessment, brief treatment and crisis stabilization for at risk high school students
•Facilitate rapid access to ongoing mental health services
•Provide consultation to school personnel as needed
•Provide individual and group counseling services
•Act as a liaison and member of high school multidisciplinary team to represent and coordinate academic, support and crisis intervention services for students
•Coordinate all student support services and interventions with the high school administrations and counseling department
•Coordinate care with agencies including DCF, etc
•Attend IEP, 504 and Student Intervention Team meetings
•Maintain clinical records in accordance with state regulations
•Establish and maintain standards for student behavior
•Assess and evaluate each student’s growth in academic, social and emotional standards
•Assign responsibilities to program staff and interns
•Communicate with parents and school counselors on student progress
•Maintain professional competence through in-service education activities provided by the district, and/or in self-selected professional growth activities
•Perform assigned tasks and duties as assigned by the Director of Special Education or the MHS Principal
