JOB DESCRIPTION
Summary
Under the manager of and in collaboration with the RN, performs responsibilities exercising initiative, judgment and independent decision-making within the scope of the State Nurse Practice Act, hospital policy and other governing organizations.
Job Duties

· Demonstrates adaptability and flexibility to changing situations including cares, medication and behaviors.*
· Identifies changes and trends in resident status. Individualizes care plans for residents depending on needs. Monitors resident outcomes and effectiveness of plan of care. Documents all findings.*
· Practices safety and infection control measures. Completes treatments, med administration and equipment use following established standards. Responds appropriately to emergency situations.*
· Observe for resident/family learning needs. Plans and coordinates with other disciplines (P.T., Pharmacist, etc.) to meet learning needs of residents. Teaches, counsels and reinforces residents and family members.*
· Assumes accountability and responsibility for quality of resident care provided. Demonstrates initiative and good judgment in all situations.*
· Fosters support and team building with residents & families, RCA's, and manager.*

Required Education and Experience
· Minnesota Licensure as a Licensed Practical Nurse
· Basic Life Support/CPR certification
Preferred Education and Experience
· Experience in Assisted Nursing Facility

· Demonstrate the knowledge and skills necessary to provide care to the resident population served in the Long Term Care facility.
· Demonstrate knowledge of the principles of growth, development and aging, from adolescence through the end of life, including death and dying.
· Assess and interpret data relating to the resident's status in order to identify the resident's age specific needs.
· Demonstrates ability to delegate work to other staff (RCA's) and maintain RCA accountability.

Position Type/Expected Hours of Work
This is a casual position, and hours worked are Monday -Friday from 8:30am-5pm as needed.
Supervisory Responsibility
Position has supervisory responsibilities of the RCA's.
Salary : $39 - $63
