Summary

Assist in the coordination of care of patients from pre-admission to discharge. This position requires a high degree of written and verbal communication with the public, clients, families, medical staff, ancillary departments and administration. Other major job functions include but are not limited to : collection of quality data; discharge planning; monitoring variances from expected outcomes. Conducts key event analysis. Assists with Utilization Review as assigned.

Job Duties

  • Acts as an advocate for patients and families to support their needs and rights.
  • Responsible for investigating and reporting all Vulnerable Adult incidents.
  • Compiles all required documentation and provides written communication to state agencies.
  • Facilitates and demonstrates interdisciplinary collaboration to promote patient centered care.
  • Conducts multidisciplinary discharge / transition planning activities three days per week.
  • Coordinates swing bed placement with social services both internal and external to the health system following Medicare and other health insurance guidelines.
  • Provides swing bed reports on a Quarterly basis.
  • Participates in care planning and discharge planning for acute care patients.
  • Assists in the finalization of complex discharge plans with patient / family.
  • Identifies and refers to community agencies for emotional, physical, psychosocial or spiritual health needs for both inpatients and outpatients.
  • Identifies pre and post hospitalization patient needs, when applicable.
  • Identifies inpatient needs and refers to appropriate service within and outside the facility as needed; includes but not limited to social services, physical & occupational therapy, dietician, etc.
  • Supports nursing staff in writing referrals.
  • Communicates with physicians regarding patient discharge and assists in developing discharge plans.
  • Serves on committee, teams and task force groups as assigned.

Required Education and Experience

  • Must have a degree in nursing from an accredited school and be currently licensed in the state of MN.
  • Must have at least five years nursing experience with at least one year critical care / ER experience preferred
  • Experience in CAH Conditions of Participation and experience in statistical data collection and reporting.
  • Experience in Utilization Management / Discharge planning experience preferred
