ACO Case Manager (RN) Position in North Fort Myers
We are seeking a full-time ACO Case Manager to join our team at Millennium Physician Group. This role involves providing direct services to assist patients in achieving their highest level of wellness.
About the Role:
The ACO Case Manager is responsible for coordinating patient care, linking clients with healthcare services, and advocating for timely and coordinated access to necessary health and support services.
Key Responsibilities:
- Assessment, planning, coordination, advocacy, education, monitoring, and accurate documentation of patient care
- Demonstrating knowledge of growth and development to interact with age-specific patient populations
Requirements:
- Active and clear license as a Registered Nurse in the State of Florida
- 3 years of clinical experience with 1 year of case management experience preferred
Preferred Skills:
- Strong computer skills, especially in Excel
- Knowledge of EMR systems; Athena knowledge preferred but not required
