About the Role:
Millennium Physician Group is seeking a compassionate and experienced Discharge Planner to join our team. As a Discharge Planner, you will play a crucial role in ensuring a smooth and safe transition for patients from the hospital to home. You will work closely with patients, families, and healthcare providers to assess individual needs, coordinate care, and facilitate access to necessary resources.
Responsibilities:
- Conduct regular hospital visits to meet with patients referred for discharge planning.
- Assess patient needs through medical record review, interviews, and physical examinations.
- Develop individualized care plans to address patient needs and promote safe home transitions.
- Coordinate with healthcare providers, social workers, and other professionals to ensure seamless care coordination.
- Facilitate access to home health services, durable medical equipment, and community resources as needed.
- Educate patients and families on self-care, medication management, and other relevant topics.
- Maintain accurate and timely documentation of patient interactions, assessments, and care plans.
- Adhere to all regulatory requirements and company policies.
- Continuously evaluate and improve discharge planning processes to enhance patient outcomes.
Qualifications:
- Active LPN license or Social Worker license
- Minimum of 2 years of experience in healthcare, preferably in discharge planning or care coordination
- Strong understanding of patient care protocols, medical terminology, and the healthcare system
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Proficiency in electronic health records (EHR) and other relevant software
- Valid driver's license and reliable transportation
