Millennium Physician Group is seeking to hire a full-time / Patient Navigator to join our Imaging Team. We are looking for a new team member who is -positive, energetic, able to multi-task, and loves patient interaction. We want someone who supports our top initiative of ensuring an excellent patient experience!
Our Patient Navigator will be guiding patients through the complex healthcare system and advocate for them. S/He will assist our patients make appointments, assuring their attendance, and connecting them to support services.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Calls all patients with imaging results within 48 business hours after testing performed.
- Checks PHI release form and ensures results are delivered in patients preferred method.
- Leaves only normal results on answering machine or voicemail if patient has authorized MPG to do so.
- Asks for call back from any patients with abnormal results and will leave direct phone number.
- Schedules patients for additional imaging for first available at patients' convenience.
- Arranges for patients with abnormal results to discuss with reading radiologist if patients wishes.
- Follows up with ordering doctor office for any additional testing needed and will enter orders if ordered by a MPG provider.
- Updates or fills out any appointment cards as needed to ensure patient receives correct information.
- Schedules breast procedures recommended by reading radiologist.
- Contacts non-MPG providers to place orders as needed.
- Follows company Standards of Behavior and Code of Conduct.

Required Knowledge and Experience
- Has previous verifiable experience working in Imaging Center and understands Imaging exams and procedures performed in this setting.
- Minimum 1 year of experience as a patient navigator or related experience in healthcare setting;
- Excellent interpersonal skills;
- Excellent time management and organizational skills;
- Ability to write professional reports and correspondence;
- Ability to work independently and as part of a team;
- Ability to identify resources and utilize problem solving skills in order to meet patient's individual needs;
- Ability to strictly adhere to HIPAA laws.
