POSITION OVERVIEW
Are you a compassionate and dedicated Nurse Practitioner (NP) looking for a chance to create a significant impact on the health and well-being of individuals seeking weight management support? Join our team at Bucks Health and Wellness, where you will work collaboratively with a physician to deliver comprehensive care and guidance to patients on their weight management journey.
KEY RESPONSIBILITIES
- Collaborate closely with the supervising physician to develop and implement personalized weight management plans for patients.
- Conduct comprehensive health assessments, including medical history, physical exams, and diagnostic tests, to evaluate patients' eligibility for weight management programs.
- Provide counseling, education, and support to patients, helping them set realistic goals and adopt healthy lifestyle changes.
- Monitor patients' progress, adjust treatment plans as needed, and offer ongoing encouragement and motivation.
- Coordinate care with registered dietitians, exercise specialists, and behavioral health professionals to ensure a multidisciplinary approach to weight management.
- Document patient care accurately and maintain up-to-date medical records.
QUALIFICATIONS
- Valid Nurse Practitioner (NP) license in Wisconsin and national certification.
- Master's or Doctoral degree in Nursing (MSN or DNP).
- Previous experience in weight management or primary care preferred.
- Strong communication and counseling skills.
- Empathy, patience, and a passion for helping patients achieve their health goals.
- Ability to work collaboratively with physicians and other healthcare professionals.
- Knowledge of evidence-based weight management strategies and practices.
