Job Description :
Mind and Mobility is seeking a Licensed Practical Nurse (LPN) to join our team as a Community Liaison. In this role, you will act as a key point of contact for clients, caregivers, and community partners to ensure high-quality care and service delivery. Key Responsibilities :
- Conduct start-of-care processes for new clients and annual client reassessments
- Conduct home consultations and safety evaluations
- Perform monthly quality assurance (QA) calls
- Conduct employee supervisory visits
- Provide on-the-job training and competency evaluations for caregivers
- Monitor and assess client condition changes to ensure appropriate care
- Participate in community events and marketing initiatives as needed
- Assist with pairing caregivers with clients according to their evaluations
