Job Description: We are seeking a skilled and compassionate PRN Home Health Physical Therapist to join our dynamic team. As a PRN Physical Therapist, you will provide comprehensive physical therapy services to patients in their homes. Responsibilities:
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Provide direct physical therapy services, including therapeutic exercises, manual therapy, and functional training.
- Monitor and evaluate patient progress, modifying treatment plans as necessary.
- Educate patients and their families on treatment goals, home exercise programs, and safety measures.
- Document patient care and progress accurately and timely in compliance with company and regulatory standards.
- Collaborate with multidisciplinary team members to ensure optimal patient care.
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in continuing education opportunities.
Qualifications:
- Valid state license to practice as a Physical Therapist.
- Graduation from an accredited Physical Therapy program.
- Minimum of 1 year of experience in home health care (preferred).
- CPR certification.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Valid driver's license and reliable transportation.
