We are currently seeking a compassionate, skilled, and experienced Geriatric Care Manager to join our growing team. This position will be responsible for overseeing and managing the care of elderly clients across 8 distinct regions via telehealth, in-person, and collaboration existing care management team members. Job Overview
The Geriatric Care Manager will serve as the primary point of contact for families, clients, and caregivers, ensuring a personalized, holistic approach to elderly care. The successful candidate will work collaboratively with families, healthcare providers, and our caregiving team to create and implement individualized care plans for our senior clients.
Key Responsibilities
- Conduct initial assessments and care plan development for new clients.
- Oversee the coordination of all non-medical care services for clients.
- Monitor client progress and provide recommendations for adjustments to care plans as needed.
- Communicate with families, ensuring they are informed and involved in care decisions.
- Serve as a liaison between the client, caregivers, and other healthcare professionals, facilitating smooth communication.
- Provide guidance and support to the caregiver team to ensure the highest standards of care.
- Address any issues, concerns, or emergencies that arise with clients, caregivers, or families.
- Maintain up-to-date records on client care plans and progress.
- Advocate for client needs and rights, ensuring the best possible quality of life.
- Ensure compliance with all state and federal regulations regarding non-medical home care services.
Qualifications
- Bachelor’s degree in Social Work, Nursing, Gerontology, or a related field (required).
- Certification in Geriatric Care Management (CGCM) preferred.
- Experience with care management, social work, or related fields.
- Strong knowledge of senior care services, community resources, and healthcare systems.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
- Compassionate, patient, and empathetic approach to senior care.
- Ability to travel between assigned regions as required.
Regions Covered
We provide in-home and outpatient physical therapy services in 3 counties of S. Florida and non-medical home care across 30 counties in Florida and Michigan. Applicants must be willing to travel within there local region to meet clients, families, and caregivers. Regions will be assigned based on location preferences and availability.
